There are several agencies that may request evaluations of individuals.  For example, the Judicial System may require a mental health and substance abuse evaluation as part of the requirements for probation.  An evaluation is required for various types of treatment programs and benefits/services.  Please check with your referring entity to determine whether they are contracted with specific evaluators before seeking an ordered evaluation.  If you are a representative from a state or community agency, please fill out a request for appointment specifying the type of evaluation needed and the purpose of the request.

 

​Evaluations are provided as requested by a provider.  Evaluations consist of a client interview, testing, and collateral information collection.  Once the evaluation is complete, a report is written providing a diagnosis (if one is identified) and recommended treatment.
